What do you need to bring to your doctor’s appointment?
Please remember to bring to your appointment:
- Medicare, DVA, Pensioner or Concession Card
- Private Health Insurance (if applicable)
- List of any medications or allergies or recent test results
- New patients – Arrive 10 minutes early to fill in a new patient and medical history form
By providing up to date information, it allows our staff to better assess your medical needs and provide you with the most appropriate medical advice and treatment.

FEE INFORMATION
We are a Bulk Billing medical practice for patients who meet the eligibility criteria.
PRIVATE HEALTH INSURANCE
Patients with Private Health Insurance cover and overseas visitors are required to pay their account on the day of service and claim expenses back from their insurance company.
TAC/WORKCOVER
TAC/Workcover patients will be required to pay fees until their account/claim number is established. Please see our friendly reception staff for further information.
HOW TO PAY
Please pay your outstanding account at the end of your appointment at reception. You can conveniently pay via:
